I found him!! I found my doc.
He is starting a new practice which will be up and running very soon. I may even be able to have my follow up with him instead of the ex-partner of his. I'm so happy and relieved. It will be a tad out of the way, very small price to pay. : - )

Glad you found him. Hope the first appointment was okay.
I feel really bad for them too, but I don't think Doc was at liberty to say anything. Today the lady from the hospital told me my doc was 'in breach of contract' and that was why they had no info, but she did give me his phone number (new practice).
Damn, woman. Look at your clustermap go!
